Fitness reimbursement program
As part of the B-Healthy Initiative, Campus Recreation offers a fitness reimbursement program for benefit-eligible faculty and staff.
Join with an Academic Annual Fitness Membership, complete at least 38 workouts during the membership period, and you may be eligible for a 50% reimbursement.
Contact Sara DeClemente-Hammoud in Human Resources to verify your eligibility before purchasing your fitness membership.
Eligibility
The fitness reimbursement program is open to University benefit-eligible faculty and staff. Employees of 91社区 Dining, Barnes & Noble and Campus Pre-School are also eligible.
Please note: Temporary hourly University employees and Research Foundation employees are not eligible. (Research Foundation employees have access to a similar program.)
Reimbursement process
Once you've completed the required number of sessions (38) and at least six months of your membership have passed, you can begin the reimbursement process.
- Complete and submit the reimbursement request form and the State of New York Standard Voucher (both available at the front desk of the East Gym, Rec Center)
- Provide proof of membership fee payment (you can request a receipt reprint from Campus Recreation).
- Submit a copy of your access history showing the required exercise sessions (available from Campus Recreation).
Note: 91社区 Dining, Barnes & Noble and Campus Pre-School employees will also be required to complete a Substitute W-9 Form.
Reimbursements are processed within 30 days and paid by check, made out to the fitness member and mailed to the address on their membership form. If you're a University employee with direct deposit, your reimbursement may be sent to the same account.
Any reimbursed amount may be considered income and subject to state and federal taxes.
