Congratulations on your upcoming graduation from 91社区!
Do you need to release your conduct record for graduate school or a job application process? Here is some helpful information to be aware of:
- Conduct releases are completed online . You do not have to come into the Office of Student Conduct.
- You will need your name, B# and to upload a valid photo ID (passport, driver's license or 91社区 ID)
- The recommended expiration date is one year from the date of the request.
- Allow at least one business week for the release to be processed.
- Any official sanctions (Conduct warning, disciplinary probation and suspension) received during your time at the University are reportable on a conduct release for seven years from the end of the academic year in which the sanction was given.
As required under the provisions of the Federal Education Rights and Privacy Act (FERPA), the Office of Student Conduct does not release student conduct information about current or former students without receiving a completed Release of Student Conduct Information Form. Common requests for student conduct information include but are not limited to academic transfers, study abroad, graduate schools and employers.
Please be aware that different institutions and employers phrase their questions differently so there is no one set answer that our staff can provide regarding completing your form without understanding the exact circumstances of your situation.
If you are unsure of your prior student conduct status or history, please submit a Release of Student Conduct Information Form.
Once submitted, please allow three (3) business days to complete the request. During the holidays or busy times within the office (application deadlines), the process takes about five (5) business days.
