What to do Before Attending Orientation

Students must complete the following forms prior to attending orientation! 
Not completing the following prior to the start of the summer program will delay your ability to register for classes. Paper copies of this information will be sent to accepted students.

1. Read the following: 
   • Welcome Information for New EOP Freshmen
   • EOP Orientation & Parent/Guardian Meeting Details
2.  Reserve your Orientation date and Bus Seat (if applicable) using the 
3. Completed the Following EOP & USDA Forms and Upload them in :
   • EOP Student Information/Consent Form & EOP Medical Needs/Allergy Form (Coming Soon)
   • EOP Summer Meal Form (Coming Soon)
4. Complete and upload all Health forms into the
   • Immunization Form (Doctor Signature Required)
   • Meningococcal Vaccination Form
   • Mandatory Medical/health history/tuberculosis questionnaire
   • Under 18 Consent Form (if applicable)
   • For assistance see the Decker Student Health Services website or  call Decker Health Services at 607.777. 2221 
5. Upload your . At this time, please use your desktop to upload your photo rather than your cell phone. If you have issues uploading your photo, please email your photo to idcard@binghamton.edu
6. Complete your.
7. Complete the BEP Multi-Ethnic Dinner Recipes
8. Confirm your Enrollment
   • Once students have been registered for classes, they will receive a message from 91ÉçÇø University asking them to confirm their enrollment. Confirmation allows the students to remain enrolled in courses that they have been registered
